Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents at the facility
• Administer medications and treatments accurately and document appropriately
• Perform thorough assessments and communicate changes in patient condition to the care team
• Collaborate with nursing staff and interdisciplinary team members to create and follow care plans
• Support safe infection control practices and maintain a clean, professional environment
• Mentor and support aides and newer staff to deliver consistent, high-quality care

Qualifications

• Active Licensed Practical Nurse (LPN) license valid in New Hampshire
• Current Basic Life Support (BLS/CPR) certification or willingness to obtain promptly
• Comfortable administering medications, charting electronically, and following care plans
• Excellent communication skills and a team-first attitude
• Experience in a clinical or long-term care setting is a plus but passion and good clinical judgment count

Education

• Diploma or certificate from an accredited LPN/Vocational Nursing program

Knowledge & Skills

• Medication administration and documentation best practices
• Patient assessment, vital signs monitoring and reporting
• Basic wound care and infection prevention techniques
• Electronic health record charting and accurate documentation
• Strong interpersonal skills for interacting with residents, families and colleagues
• Time management and prioritization during busy shifts
