Roles & Responsibilities

• Provide compassionate, patient-centered care during overnight shifts, ensuring safety and comfort
• Administer medications and treatments accurately and document all care in the medical record
• Monitor and assess patient conditions, promptly reporting changes to the RN or provider
• Collaborate with the interdisciplinary team to support care plans and positive outcomes
• Perform timely charting and handoffs to ensure continuity of care during transitions
• Support residents with activities of daily living and promote a dignified environment
• Respond effectively to emergencies and follow facility protocols to maintain patient safety

Qualifications

• Active LPN license in New Hampshire or eligibility to obtain state licensure
• Current BLS/CPR certification (or willing to obtain prior to start)
• Comfortable working overnight shifts and adapting to a planned transition to evenings
• Strong clinical assessment skills, attention to detail, and reliable documentation practices
• Good communication and teamwork skills; ability to collaborate with nurses, providers, and support staff
• Ability to pass standard background checks and meet facility onboarding requirements

Education

• Graduate of an accredited practical nursing (LPN/LVN) program; high school diploma or GED required

Knowledge & Skills

• Medication administration and basic nursing procedures with accuracy
• Patient assessment, vital signs monitoring, and recognizing clinical changes
• Familiarity with electronic medical records and clear documentation practices
• Infection control principles and safe patient handling techniques
• Time management skills to prioritize tasks in a fast-paced overnight environment
• A caring bedside manner and the ability to support residents emotionally and physically
