Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in accordance with facility policies
• Administer medications and treatments safely and accurately
• Assess, monitor, and document patient status including vital signs and changes in condition
• Collaborate with interdisciplinary team members to create and update individualized care plans
• Support activities of daily living and promote patient comfort and dignity
• Educate patients and families on care plans and safe home strategies
• Maintain accurate charting in the electronic medical record and ensure continuity of care

Qualifications

• Current Licensed Practical Nurse (LPN) license; must be valid in North Carolina
• Basic Life Support (BLS/CPR) certification required or obtain upon hire
• Comfortable working rotating 12-hour night shifts
• Local to Pembroke area or able to commute reliably (LOCAL ONLY)
• Strong communication skills and a team-focused attitude
• Previous LPN experience in long-term care or similar setting preferred

Education

• LPN diploma, vocational nursing certificate, or equivalent practical nursing education

Knowledge & Skills

• Skilled in medication administration, resident assessment, and wound care basics
• Proficient with documentation and charting best practices
• Knowledge of infection control and safety protocols
• Ability to prioritize tasks in a fast-paced clinical environment
• Collaborative approach to working with nurses, support staff, and leadership
