Roles & Responsibilities

• Assess speech, language, cognitive-communication, and swallowing function using evidence-based tools
• Develop and implement individualized treatment plans that target meaningful outcomes
• Document progress and outcomes in the facility electronic health record in a timely, compliant manner
• Collaborate with interdisciplinary team members to coordinate care and support safe discharge plans
• Provide education and training to patients, families, and caregivers to promote carryover and independence
• Participate in patient-centered rounds and contribute to quality improvement initiatives

Qualifications

• Current Massachusetts SLP licensure or ability to obtain prior to start
• ASHA Certificate of Clinical Competence (CCC-SLP) preferred
• Valid BLS/CPR certification as required by facility
• Experience working with adult and geriatric populations in rehab or skilled nursing settings preferred
• Ability to pass background checks and required health screenings

Education

• Master's degree in Speech-Language Pathology

Knowledge & Skills

• Skilled in dysphagia assessment and treatment, voice and cognitive-communication therapy
• Strong clinical documentation and goal-writing skills
• Comfortable using facility EHR systems and following regulatory documentation standards
• Excellent communication and teamwork skills with patients, families, and clinical colleagues
• Ability to prioritize caseloads and provide efficient, high-quality care
