Roles & Responsibilities
- Assess speech, language, cognitive-communication, and swallowing function using evidence-based tools
- Develop and implement individualized treatment plans that target meaningful outcomes
- Document progress and outcomes in the facility electronic health record in a timely, compliant manner
- Collaborate with interdisciplinary team members to coordinate care and support safe discharge plans
- Provide education and training to patients, families, and caregivers to promote carryover and independence
- Participate in patient-centered rounds and contribute to quality improvement initiatives
Qualifications
- Current Massachusetts SLP licensure or ability to obtain prior to start
- ASHA Certificate of Clinical Competence (CCC-SLP) preferred
- Valid BLS/CPR certification as required by facility
- Experience working with adult and geriatric populations in rehab or skilled nursing settings preferred
- Ability to pass background checks and required health screenings
Education
- Master's degree in Speech-Language Pathology
Knowledge & Skills
- Skilled in dysphagia assessment and treatment, voice and cognitive-communication therapy
- Strong clinical documentation and goal-writing skills
- Comfortable using facility EHR systems and following regulatory documentation standards
- Excellent communication and teamwork skills with patients, families, and clinical colleagues
- Ability to prioritize caseloads and provide efficient, high-quality care
