Roles & Responsibilities

• Provide patient-centered occupational therapy evaluation and intervention to support functional independence
• Develop and implement individualized treatment plans based on assessment, goals, and best-practice standards
• Collaborate with interdisciplinary team members to coordinate care and optimize patient outcomes
• Document accurately and timely in the electronic medical record, including progress notes and discharge summaries
• Educate patients and caregivers on strategies, adaptive equipment and home safety to promote carry-over
• Participate in team meetings, quality improvement activities and skill-building to enhance clinical practice

Qualifications

• Current, active Occupational Therapist license in the state of Vermont (or eligibility and willingness to obtain)
• Certified Occupational Therapy Assistant (COTA) candidates considered for the COTA role with appropriate state licensure
• Basic Life Support (BLS) / CPR certification required or willing to obtain
• Previous experience in rehab, outpatient or restorative care is highly valued
• Ability to work all scheduled days and maintain professional communication with the care team
• Strong clinical reasoning, documentation skills, and a compassionate patient-first approach

Education

• Occupational Therapist: Bachelor's, Master's or OTD in Occupational Therapy (as required by state licensure)
• COTA: Associate degree or accredited Occupational Therapy Assistant program completion

Knowledge & Skills

• Skilled in ADL/IADL retraining and adaptive equipment recommendations
• Knowledge of therapeutic modalities, orthotics, splinting and activity analysis
• Proficient documentation and time-management to meet productivity and compliance standards
• Excellent communication and interdisciplinary collaboration skills
• Comfortable with caseload prioritization and working in a fast-paced rehab environment
