Roles & Responsibilities

• Evaluate patients' functional abilities and design individualized treatment plans that make daily life easier
• Deliver evidence-based occupational therapy interventions to improve mobility, independence, and ADLs
• Document patient assessments, progress notes, and discharge plans in line with facility and regulatory guidelines
• Collaborate with PT, SLP, nursing, and case management to coordinate holistic patient care
• Educate patients and families on therapy goals, safety, and home programs to support ongoing recovery
• Maintain professional standards and ensure compliance with state, federal, and facility policies

Qualifications

• Active California Occupational Therapy license in good standing (required)
• Bachelor’s, Master’s, or Doctorate degree in Occupational Therapy from an accredited program
• Previous rehabilitation or skilled nursing experience preferred
• Strong communication, assessment, and documentation skills
• Ability to manage a caseload efficiently and work independently as well as part of a team
• Willingness to work in a collaborative interdisciplinary environment

Education

• Bachelor’s, Master’s, or Doctorate in Occupational Therapy from an accredited program

Knowledge & Skills

• Occupational Therapy clinical assessment and treatment planning
• Skilled documentation and progress note writing aligned with regulatory standards
• Evidence-based interventions for mobility, ADLs, and functional independence
• Interdisciplinary collaboration with PT, SLP, nursing, and case management
• Strong communication skills with patients, families, and facility staff
• Caseload management and prioritization in a skilled nursing setting
