Roles & Responsibilities
- Evaluate patients' functional abilities and design individualized treatment plans that make daily life easier
- Deliver evidence-based occupational therapy interventions to improve mobility, independence, and ADLs
- Document patient assessments, progress notes, and discharge plans in line with facility and regulatory guidelines
- Collaborate with PT, SLP, nursing, and case management to coordinate holistic patient care
- Educate patients and families on therapy goals, safety, and home programs to support ongoing recovery
- Maintain professional standards and ensure compliance with state, federal, and facility policies
Qualifications
- Active California Occupational Therapy license in good standing (required)
- Bachelor’s, Master’s, or Doctorate degree in Occupational Therapy from an accredited program
- Previous rehabilitation or skilled nursing experience preferred
- Strong communication, assessment, and documentation skills
- Ability to manage a caseload efficiently and work independently as well as part of a team
- Willingness to work in a collaborative interdisciplinary environment
Education
- Bachelor’s, Master’s, or Doctorate in Occupational Therapy from an accredited program
Knowledge & Skills
- Occupational Therapy clinical assessment and treatment planning
- Skilled documentation and progress note writing aligned with regulatory standards
- Evidence-based interventions for mobility, ADLs, and functional independence
- Interdisciplinary collaboration with PT, SLP, nursing, and case management
- Strong communication skills with patients, families, and facility staff
- Caseload management and prioritization in a skilled nursing setting
