Roles & Responsibilities

• Provide compassionate direct patient care consistent with LPN scope of practice
• Administer medications and document treatments accurately and on time
• Monitor and record vital signs and report changes to the care team
• Support nursing assessments and contribute to individualized care plans
• Collaborate with RNs, physicians, and interdisciplinary staff to coordinate care
• Educate patients and families on procedures, medications, and discharge instructions
• Maintain a safe, clean environment and adhere to infection prevention protocols
• Accurately document patient care in the electronic health record

Qualifications

• Active Licensed Practical Nurse (LPN) license valid for practice in New Hampshire
• Current BLS/CPR certification or willingness to obtain prior to start
• Completion of an accredited practical nursing program
• Up-to-date immunizations and ability to meet facility health requirements
• Strong communication skills and a patient-first attitude
• Previous LPN experience preferred but new grads from accredited programs may be considered

Education

• Graduate of an accredited Practical Nursing program (LPN diploma or certificate)
• High school diploma or GED required

Knowledge & Skills

• Proficient in medication administration and basic nursing procedures
• Comfortable with vital sign monitoring, wound care, and patient assessments
• Familiarity with electronic health record documentation and charting
• Strong clinical observation and prioritization skills
• Excellent interpersonal skills for patient and team communication
• Basic knowledge of infection control standards and safety protocols
