Roles & Responsibilities

• Provide compassionate direct patient care under supervision, assisting with activities of daily living
• Monitor and report changes in patient condition to the nursing team promptly
• Assist with safe transfers, mobility, and toileting to preserve patient dignity
• Maintain accurate documentation of care and observations
• Support infection control and facility safety standards at all times
• Collaborate with nurses and interdisciplinary team members to ensure continuity of care

Qualifications

• Required: Valid Licensed Nursing Assistant (LNA) certification (VT or NH as applicable)
• Current basic patient care skills and comfort working in a long-term care/facility setting
• Preferred: Current CPR/BLS certification
• Reliable, punctual, and able to pass facility background and health screenings

Education

• LNA certification/license
• High school diploma or equivalent preferred

Knowledge & Skills

• Understanding of basic patient care principles and safety protocols
• Good communication and teamwork skills with patients, families, and staff
• Ability to follow care plans and document accurately
• Attention to detail and strong observational skills
