Roles & Responsibilities

• Provide compassionate, hands-on patient care including bathing, dressing, feeding, and mobility assistance
• Monitor and report changes in patient condition to nursing staff promptly
• Assist with safe transfers, ambulation, and fall prevention practices
• Support documentation of care, vitals, and patient status in the electronic chart
• Collaborate with the interdisciplinary team to deliver person-centered care
• Maintain infection control standards and a safe environment for patients and staff

Qualifications

• Current Licensed Nursing Assistant (LNA) credential for NH or VT, or ability to obtain prior to start
• Current BLS/CPR certification (or willingness to obtain)
• Comfortable working overnight shifts and providing direct patient care
• Strong communication skills and a team-oriented attitude
• Prior experience in long-term care, rehab, or med-surg settings is helpful but not required

Education

• High school diploma or equivalent; completion of a state-approved nursing assistant training program preferred

Knowledge & Skills

• Fundamental patient care techniques and safe body mechanics
• Basic knowledge of infection prevention and PPE use
• Ability to take and record vital signs accurately
• Documentation and time management skills
• Compassion, patience, and effective communication with patients and families
