Shift Type: Monday-Friday, 9:00 AM - 5:30 PM (Day shift), 5x8 with block scheduling; every-other-weekend coverage required

Candidate Pay Rate: $57 per hour

Roles & Responsibilities
- Coordinate comprehensive case management and discharge planning to ensure safe, timely transitions of care
- Conduct psychosocial assessments and develop individualized plans of care
- Perform utilization review and prior authorization activities to support appropriate care delivery
- Document thoroughly and accurately in Epic to reflect patient goals, teaching, and care coordination
- Collaborate with internal and external team members, including community resources, to arrange home care, DME, and follow-up services
- Facilitate care conferences and communicate clearly with patients, families, and the care team
- Support protective services processes including guardianship and emergency detentions when needed
- Provide education to patients and caregivers to promote successful discharges and continuity of care
Qualifications
- Current Licensed Clinical Social Worker (LCSW) license in hand; Oregon licensure required or ability to obtain Oregon LCSW
- Minimum 2 years of case management experience, with hospital experience required
- Proficiency with Epic or similar electronic charting systems
- Comfort floating between units and working block schedules
- Ability to cover every-other-weekend rotation and occasional unit float (per client note)
- Strong documentation, communication, and care coordination skills
- If candidate is a nurse, Oregon RN license is required
Education
- Master of Social Work (MSW) preferred; educational background consistent with LCSW licensure
Knowledge & Skills
- Deep knowledge of case management, utilization review, and discharge planning
- Familiarity with community programs such as Medicaid, Medicare, HCBS, and Agency on Aging
- Understanding of advance directives, protective services, guardianship, and power of attorney
- Experience with complex disease processes including dementia, CHF, COPD, diabetes, and psychiatric disorders
- Skilled at conducting assessments, developing/revising care plans, and establishing measurable goals
- Ability to navigate appeals and denials, benefits eligibility, and prior authorization workflows
- Strong cultural competency and adherence to NASW Code of Ethics
