Roles & Responsibilities

• Assess patient functional abilities and create individualized, evidence-based treatment plans
• Deliver hands-on therapy, therapeutic exercise, and mobility training to promote independence
• Collaborate with nursing, physicians, and restorative teams to coordinate patient-centered care
• Document patient progress clearly and timely in the electronic medical record
• Educate patients and families about home programs, fall prevention, and safe mobility
• Participate in discharge planning and continuity of care to ensure successful transitions
• Contribute to a culture of safety and quality by following facility policies and best practices

Qualifications

• Licensed Physical Therapist in Maine or eligible to obtain Maine licensure
• Current BLS/CPR certification (or willing to obtain prior to start)
• Comfortable working in a fast-paced contract setting with a flexible, positive attitude
• Experience in outpatient/rehabilitation settings preferred but new grads with strong skills are welcome
• Strong clinical reasoning, documentation, and interpersonal communication skills

Education

• Graduate of an accredited Physical Therapy program (DPT preferred)

Knowledge & Skills

• Skilled in therapeutic exercise, manual therapy, gait training, and ADL retraining
• Familiarity with electronic medical records and timely documentation
• Ability to assess and modify treatment plans based on patient progress
• Excellent teamwork, patient education, and family communication skills
• Knowledge of safety standards, fall prevention, and infection control practices
