Roles & Responsibilities

• Provide high-quality primary care to adult patients, delivering evidence-based diagnosis and treatment
• Manage chronic conditions and coordinate care plans to improve long-term outcomes
• Perform thorough assessments and document clinical encounters clearly in the EHR
• Collaborate with behavioral health and substance use treatment teams at the facility
• Prescribe and manage medications responsibly, including monitoring for efficacy and safety
• Educate patients about preventive care and health maintenance to support healthier lives
• Participate in facility meetings and quality improvement initiatives to enhance patient care

Qualifications

• Active or eligible California medical license (MD or DO) — help available with licensure steps
• Board-certified or board-eligible in Family Medicine or Internal Medicine
• Current BLS certification; ACLS preferred
• DEA registration or ability to obtain DEA privileges for controlled substances
• Clinical experience in primary care; experience with substance use disorder or correctional health is a strong plus
• Strong interpersonal skills with proven ability to work in a multidisciplinary team

Education

• MD or DO degree and successful completion of an accredited residency in Family Medicine or Internal Medicine

Knowledge & Skills

• Skilled in comprehensive primary care assessment, diagnosis, and management
• Comfortable working with patients who have substance use and behavioral health needs
• Familiarity with EHR systems and clear, concise clinical documentation
• Excellent communication skills for patient education and interdisciplinary collaboration
• Ability to prioritize tasks and manage a clinic schedule efficiently
