Roles & Responsibilities

• Provide skilled physical therapy evaluations and treatments in patients’ homes, tailoring plans to individual needs
• Develop and update patient-centered care plans focused on mobility, safety, and independence
• Perform home environment assessments and recommend adaptive equipment or safety modifications
• Document clinical findings and progress thoroughly and professionally (Epic preferred)
• Communicate clearly with patients, families, and the interdisciplinary care team to coordinate effective home care
• Participate in Trinity specialty competency testing and skills checklists to maintain high clinical standards

Qualifications

• Valid Physical Therapy license (ability to verify licensure required)
• BLS certification required
• Home health experience preferred but thoughtful, motivated new clinicians are welcome
• Epic experience preferred, but not required
• Ability to complete Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
• Must meet required health screenings and immunizations (MMR, Hepatitis B, Varicella, Tdap, COVID-19, Influenza, 2-step PPD or IGRA) and pass required drug screen and fit test

Knowledge & Skills

• Strong patient assessment and clinical reasoning skills for home health populations
• Skilled in mobility, gait training, balance retraining, and functional transfers
• Comfortable performing home safety and fall risk evaluations
• Clear and timely clinical documentation; Epic experience is a plus
• Excellent communication and patient/family education skills
• Ability to work independently and manage caseloads in a community setting
