Roles & Responsibilities
- Assess patients and develop individualized treatment plans to improve mobility, reduce pain, and lower fall risk
- Provide hands-on therapy and mobility training to help patients return to daily activities and work tasks
- Document evaluations, progress notes, discharges, daily notes, insurance approvals/denials, and phone conversations accurately and promptly
- Collaborate and communicate with physicians, nurses, other rehab staff, insurance companies, patients, and families to coordinate care
- Assist with discharge planning to ensure safe transitions and continuity of care
- Support clinic operations by maintaining clean treatment areas and assisting with light housekeeping tasks as needed
- Perform other duties as requested to support patient care and clinic workflow
Qualifications
- Current, active Physical Therapist license in Wisconsin (or eligible for licensure)
- Degree in Physical Therapy (DPT or equivalent) from an accredited program
- Current BLS/CPR certification (or willingness to obtain upon hire)
- Strong patient assessment, documentation, and communication skills
- Experience treating movement dysfunction, pain management, and fall risk reduction preferred
- Comfort interacting with insurance processes including approvals and denials
Education
- Doctor of Physical Therapy (DPT) or equivalent degree
- Licensed Physical Therapist (state licensure required)
Knowledge & Skills
- Physical therapy evaluation and treatment planning
- Pain management and mobility training techniques
- Fall risk assessment and prevention strategies
- Clinical documentation and progress note best practices
- Effective communication with multidisciplinary teams and payers
- Discharge planning and care coordination
