Roles & Responsibilities

• Assess patients and develop individualized treatment plans to improve mobility, reduce pain, and lower fall risk
• Provide hands-on therapy and mobility training to help patients return to daily activities and work tasks
• Document evaluations, progress notes, discharges, daily notes, insurance approvals/denials, and phone conversations accurately and promptly
• Collaborate and communicate with physicians, nurses, other rehab staff, insurance companies, patients, and families to coordinate care
• Assist with discharge planning to ensure safe transitions and continuity of care
• Support clinic operations by maintaining clean treatment areas and assisting with light housekeeping tasks as needed
• Perform other duties as requested to support patient care and clinic workflow

Qualifications

• Current, active Physical Therapist license in Wisconsin (or eligible for licensure)
• Degree in Physical Therapy (DPT or equivalent) from an accredited program
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Strong patient assessment, documentation, and communication skills
• Experience treating movement dysfunction, pain management, and fall risk reduction preferred
• Comfort interacting with insurance processes including approvals and denials

Education

• Doctor of Physical Therapy (DPT) or equivalent degree
• Licensed Physical Therapist (state licensure required)

Knowledge & Skills

• Physical therapy evaluation and treatment planning
• Pain management and mobility training techniques
• Fall risk assessment and prevention strategies
• Clinical documentation and progress note best practices
• Effective communication with multidisciplinary teams and payers
• Discharge planning and care coordination
