Contract Type: Travel Contract
Shift Type: Day shift with call requirements (call 1-2 nights per week and 1-2 weekends per month). Specific start/end times vary by assignment and site.
Hours per Week: 36
Candidate Pay Rate: $53 per hour

Roles & Responsibilities
- Provide skilled nursing care and clinical assessments to home health, hospice, and palliative care patients
- Perform initial evaluation visits and regularly re-evaluate patient nursing needs
- Develop, implement, and revise individualized Plans of Care using the nursing process
- Prepare accurate clinical and progress documentation and communicate changes in patient condition to the interdisciplinary team
- Coordinate services and collaborate with physicians, therapists, and other care providers to support patient-centered care
- Counsel patients and families on nursing needs, symptom management, and end-of-life care options
- Participate in in-service education and supervise/teach other nursing personnel to promote best practices
Qualifications
- Active Registered Nurse (RN) licensure (Colorado) or eligibility to obtain prior to start
- Experience or comfort working in home health, hospice, or palliative care settings
- Demonstrated ability with clinical documentation and care coordination
- Willingness to participate in call rotation including nights and occasional weekends
Knowledge & Skills
- Nursing process and development of individualized Plans of Care
- Skilled nursing interventions for intermittent professional nursing needs
- Case management and care coordination across interdisciplinary teams
- Clinical documentation and progress note preparation
- Patient and family counseling, education, and support
- Supervision, teaching, and participation in in-service training
