Roles & Responsibilities

• Provide skilled nursing care and clinical assessments to home health, hospice, and palliative care patients
• Perform initial evaluation visits and regularly re-evaluate patient nursing needs
• Develop, implement, and revise individualized Plans of Care using the nursing process
• Prepare accurate clinical and progress documentation and communicate changes in patient condition to the interdisciplinary team
• Coordinate services and collaborate with physicians, therapists, and other care providers to support patient-centered care
• Counsel patients and families on nursing needs, symptom management, and end-of-life care options
• Participate in in-service education and supervise/teach other nursing personnel to promote best practices

Qualifications

• Active Registered Nurse (RN) licensure (Colorado) or eligibility to obtain prior to start
• Experience or comfort working in home health, hospice, or palliative care settings
• Demonstrated ability with clinical documentation and care coordination
• Willingness to participate in call rotation including nights and occasional weekends

• Skilled nursing interventions for intermittent professional nursing needs
• Case management and care coordination across interdisciplinary teams
• Clinical documentation and progress note preparation
• Patient and family counseling, education, and support
