Roles & Responsibilities
- Perform thorough patient assessments in the home setting to establish baseline function and needs
- Develop and implement individualized physical therapy treatment plans that promote mobility, safety, and independence
- Document all visits and clinical progress accurately in HCHB to ensure continuity of care
- Coordinate care with nurses, aides, and interdisciplinary team members to optimize patient outcomes
- Educate patients and caregivers on home exercise programs, safety, and fall prevention
- Manage a typical caseload of approximately 5–6 patient visits per day while prioritizing quality and efficiency
- Communicate changes in patient status promptly and participate in care planning and discharge preparation
Qualifications
- Minimum 2 years of clinical experience, with home health experience for therapy strongly preferred
- Home health and hospice experience is a must for this region (Franklin / Macon County)
- Active Physical Therapist license or eligible to be licensed in the state of North Carolina
- Comfortable and experienced with HCHB documentation system
- Reliable vehicle and valid driver’s license for patient visits
- Strong interpersonal and communication skills for patient and family education
Education
- Doctor of Physical Therapy (DPT) preferred or equivalent entry-level PT degree
Knowledge & Skills
- Skilled in home health patient assessment and treatment planning
- Proficient with HCHB documentation workflows
- Knowledge of home safety, adaptive equipment, and discharge planning
- Ability to manage caseloads efficiently and prioritize patient needs
- Strong clinical reasoning, patient education, and care coordination skills
