Roles & Responsibilities
- Conduct comprehensive psychosocial assessments to understand each patient’s needs and strengths
- Develop individualized, evidence-based treatment plans that support whole-person care
- Deliver therapeutic interventions across the lifespan for issues such as depression, anxiety, trauma, and substance use
- Coordinate care with primary care providers, nurses, and behavioral health colleagues to ensure integrated treatment
- Maintain timely and thorough clinical documentation to support continuity of care
- Engage with patients and families to provide education, resources, and community referrals
- Advocate for underserved populations and contribute to outreach efforts in rural settings
Qualifications
- Active LISW license in Iowa (required)
- Master’s degree in Social Work from an accredited program (required)
- Minimum of 2 years of clinical experience preferred
- Experience working in rural or medically underserved settings is a plus
- Strong interpersonal and organizational skills with excellent clinical documentation
- Comfort collaborating in interdisciplinary teams and coordinating patient-centered care
Education
- Master’s degree in Social Work from an accredited program
Knowledge & Skills
- Skilled in psychosocial assessment and treatment planning
- Proficient with evidence-based therapeutic interventions across age groups
- Experience in care coordination and interdisciplinary collaboration
- Knowledge of trauma-informed care and substance use treatment approaches
- Strong clinical documentation and record-keeping abilities
- Ability to engage and support diverse and underserved populations
