Roles & Responsibilities
- Provide compassionate direct patient care, including medication administration and wound care
- Monitor patient status and communicate changes clearly and promptly to the care team
- Document assessments, treatments and patient responses accurately and timely
- Assist with admissions, discharges and daily care planning to support continuity
- Collaborate with nurses, therapists and providers to deliver patient-centered care
- Support residents' dignity and independence through respectful, personalized care
Qualifications
- Current Licensed Practical Nurse (LPN) license — Alabama or compact eligibility recommended
- Minimum clinical experience working in long-term care or similar settings preferred
- Basic Life Support (BLS) / CPR certification (or ability to obtain prior to start)
- Up-to-date immunizations and any facility-required health screenings
- Excellent communication skills and a team-oriented attitude
Education
- Completed LPN/Vocational Nursing program and active LPN license
Knowledge & Skills
- Strong skills in medication administration and charting/documentation
- Good assessment skills and ability to recognize and escalate changes in condition
- Proficiency in basic nursing procedures common to long-term care
- Ability to work collaboratively with interdisciplinary teams
- Time management and prioritization in a busy clinical environment
