Roles & Responsibilities

• Assess patients' physical function and develop individualized treatment plans
• Provide hands-on therapy and therapeutic exercise to improve mobility and independence
• Document progress clearly and timely in the medical record using company standards
• Collaborate with occupational therapy, nursing, physicians and case managers to coordinate care
• Educate patients and families on home programs and fall-prevention strategies
• Monitor outcomes and modify plans of care to achieve measurable patient goals
• Advocate for patient-centered care and participate in team-based discharge planning

Qualifications

• Active Physical Therapist license in New Mexico or eligibility to obtain state licensure promptly
• Current BLS/CPR certification (or willingness to obtain prior to start)
• Travel/contract clinicians must meet agency credentialing requirements for New Mexico
• Comfortable working in a fast-paced rehab/contract facility and collaborating with interdisciplinary teams
• Strong clinical reasoning and patient education skills

Education

• Doctor of Physical Therapy (DPT) preferred or equivalent entry-level PT degree

Knowledge & Skills

• Skilled in evaluation, therapeutic exercise, manual therapy and mobility training
• Familiarity with common inpatient/outpatient rehab diagnoses and interventions
• Able to create clear, measurable goals and document outcomes
• Strong communication skills for patient education and interdisciplinary collaboration
• Proficiency with electronic medical records and clinical documentation best practices
