Shift Type: Evening shift, 3:00 PM - 11:00 PM; schedule varies and includes every other weekend (rotating evenings)

Candidate Pay Rate: $24 per hour

Roles & Responsibilities
- Provide compassionate, hands-on personal care and assistance with activities of daily living
- Assist with patient transfers, mobility and safe positioning to support rehabilitation goals
- Monitor and report changes in patient condition promptly to the nursing team
- Document care and observations accurately and timely in the facility charts
- Collaborate with nurses, therapists and family members to deliver patient-centered care
- Support facility routines including meal assistance, hygiene, and therapeutic activities
Qualifications
- Current LNA / Licensed Nursing Assistant certification (eligible to work in Vermont)
- Proven experience as an LNA or nursing assistant preferred
- Reliable transportation — travel assignment requires local mobility
- Weekend availability — every other weekend required
- Willingness to work evening shifts (3:00 PM - 11:00 PM) and occasional overtime
- Ability to pass background check, drug screen and required immunizations
Education
- High school diploma or GED required; completion of an approved nursing assistant training program preferred
Knowledge & Skills
- Strong patient care and bedside manner — kindness and patience go a long way
- Basic clinical skills: vital signs, ADL assistance, safe transfer techniques
- Clear, professional communication and teamwork
- Good documentation and charting habits
- Infection control practices and adherence to facility procedures
- Time management and flexibility in a fast-paced rehab/long-term care setting
