Roles & Responsibilities

• Evaluate patients' physical function and develop individualized treatment plans to restore mobility and independence
• Deliver evidence-based therapeutic interventions and manual techniques to improve strength, balance, and gait
• Document progress clearly and timely in the medical record to support continuity of care
• Collaborate with nursing, case management, and multidisciplinary teams to support discharge planning and goals
• Educate patients and families on home exercise programs and safety strategies to promote long-term recovery
• Supervise and mentor therapy assistants and aides to ensure high-quality, consistent care
• Monitor outcomes and adjust treatment plans to maximize functional gains and patient satisfaction

Qualifications

• Active or eligible Physical Therapist (PT) credential — ready to practice in the assignment state
• Experience or interest in nursing rehabilitation and working with post-acute populations
• Strong documentation skills and comfort using electronic medical records
• Basic Life Support (BLS/CPR) certification preferred or willing to obtain
• Excellent communication and teamwork skills to coordinate with interdisciplinary staff

Education

• Graduation from an accredited Physical Therapy program (DPT preferred) or equivalent

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise, and mobility training
• Knowledge of rehabilitation principles for post-acute and skilled nursing care
• Proficient in developing measurable goals and outcome-driven treatment plans
• Ability to teach and progress home exercise programs and caregiver training
• Good clinical judgment, time management, and documentation accuracy
