Shift Type: Rotating 2-week schedule with both day and evening coverage: Week 1 — Sunday 7:00 AM - 3:30 PM; Monday, Tuesday, Thursday, Friday 3:00 PM - 11:30 PM. Primary shift block is 3:00 PM - 11:30 PM. Schedule includes both day and evening shifts (day_night).
Hours per Week: 40
Candidate Pay Rate: $34 per hour

Roles & Responsibilities
- Administer medications and treatments safely and accurately according to plan
- Perform routine assessments and monitor patients for changes in condition
- Document care and communicate observations clearly in the patient record
- Collaborate with RNs, physicians, and interdisciplinary team members to support patient goals
- Provide compassionate bedside care and patient education that promotes recovery and dignity
- Assist with wound care, IV monitoring, and other routine procedures within LPN scope
- Support safe patient transitions and discharge planning through clear communication
Qualifications
- Required: Active Licensed Practical Nurse (LPN) license — ability to practice in New Hampshire preferred
- Required: Local-only availability and ability to work the posted rotating schedule (including 3:00 PM - 11:30 PM shifts)
- Preferred: Previous experience in long-term care, skilled nursing, or similar settings
- Preferred: Current BLS/CPR certification and up-to-date immunizations
- Strong communication skills and a team-oriented mindset
Education
- Graduate of an accredited Practical Nursing (LPN/Vocational Nursing) program
Knowledge & Skills
- Solid knowledge of medication administration and safe nursing practices
- Comfortable performing basic assessments, vitals, and documentation in EMR systems
- Ability to prioritize tasks in a fast-paced clinical environment
- Good interpersonal skills for patient, family, and team interactions
- Detail-oriented approach to charting, reporting, and following care plans
