Roles & Responsibilities

• Evaluate patients' functional mobility and design individualized treatment plans
• Implement evidence-based therapeutic interventions to improve patient outcomes
• Document patient progress accurately and timely in the electronic medical record
• Collaborate with interdisciplinary teams to coordinate care and discharge planning
• Educate patients and families on safe mobility, home exercise programs, and fall prevention
• Monitor and adjust treatments based on outcomes and patient response
• Lead safe transfers, gait training, and therapeutic exercise sessions to maximize independence

Qualifications

• Valid California Physical Therapist license or ability to obtain licensure in CA
• Basic Life Support (BLS/CPR) certification (current)
• Previous experience in outpatient, inpatient rehab, or acute care settings preferred
• Strong clinical assessment, manual therapy, and therapeutic exercise skills
• Excellent communication, documentation, and teamwork abilities
• Willingness to work the listed schedule including weekends as required

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor’s in Physical Therapy or related degree accepted where applicable
• Completion of a CAPTE-accredited program (or equivalent) and state licensure

Knowledge & Skills

• Skilled in developing and implementing individualized PT treatment plans
• Familiarity with common rehab modalities, gait training, and balance interventions
• Proficient with electronic medical records and accurate clinical documentation
• Knowledge of patient safety, infection control, and regulatory standards
• Strong interpersonal skills for patient education and interdisciplinary collaboration
