Roles & Responsibilities

• Evaluate patients in their homes using evidence-based assessments to establish baseline function
• Develop individualized treatment plans focused on improving mobility, function, and independence
• Provide skilled therapeutic interventions and educate patients and caregivers to support safe home care
• Document clinical findings, progress, and plans in EPIC in a timely and professional manner
• Coordinate care with the interdisciplinary team to ensure continuity and quality of care
• Complete required competency testing and skill checklists to maintain clinical standards
• Ensure compliance with immunization, respirator, and screening requirements for safe patient care

Qualifications

• Active Physical Therapy license (able to pass License/Certificate Verification)
• BLS certification required
• Comfortable documenting in EPIC (electronic medical record experience preferred)
• Willingness to complete Trinity Skills Checklist and specialty competency testing
• Ability to pass Trinity respirator evaluation, fit testing, and 10-panel drug screening
• Up-to-date vaccinations and compliance with required health screenings

Knowledge & Skills

• Strong skills in physical therapy evaluation and home-based treatment planning
• Experience or comfort with EPIC for documentation and care coordination
• Knowledge of infection control, immunization requirements, and respirator use
• Patient and family education skills to support home safety and self-management
• Competency with clinical skill verification and specialty-specific assessments
