Roles & Responsibilities

• Perform thorough home-based patient evaluations and develop individualized treatment plans
• Deliver skilled therapeutic interventions to improve mobility, function, and independence
• Document patient care accurately and promptly using HCHB
• Coordinate care with nurses, aides, and interdisciplinary team members to ensure continuity
• Manage a caseload of approximately 5–6 patients per day with professional time management
• Educate patients and caregivers on home exercise programs, safety, and fall prevention
• Communicate updates and progress clearly to referring clinicians and the care team

Qualifications

• Licensed Physical Therapist (PT) in North Carolina or eligible for licensure
• Minimum 2 years of relevant experience — home health experience preferred
• Experience working in home health and familiarity with hospice care preferred
• Comfortable and proficient with HCHB documentation system
• Reliable vehicle for patient visits and valid driver’s license

Knowledge & Skills

• Strong clinical assessment and treatment planning skills for adult and geriatric populations
• Skilled with home safety evaluations and adaptive equipment recommendations
• Excellent communication and patient/family education abilities
• Efficient scheduling and caseload management for community-based care
• Proficient documentation practices and attention to regulatory detail
