Roles & Responsibilities

• Assess and evaluate patient physical function and mobility using evidence-based methods
• Develop individualized treatment plans that improve safety, independence, and quality of life
• Deliver therapeutic interventions including exercise, manual therapy, and functional training
• Document progress clearly and timely in the patient record and communicate changes to the care team
• Collaborate with nurses, occupational therapists, physicians, and support staff to coordinate care
• Educate patients and families on home programs, fall prevention, and adaptive strategies

Qualifications

• Active California Physical Therapist license (required)
• Doctor of Physical Therapy (DPT) or equivalent degree preferred
• Current BLS/CPR certification (or willingness to obtain prior to start)
• Experience working in medical/acute or skilled nursing environments preferred
• Strong clinical assessment, documentation, and communication skills
• Ability to work Monday–Friday daytime schedule and adapt to facility needs

Education

• Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy
• Active California PT license

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise, gait training, and transfers
• Knowledge of rehabilitation principles for adult medical populations
• Comfortable with documentation in electronic health records (EHR)
• Excellent interpersonal skills and a collaborative mindset
• Ability to prioritize caseload and manage time efficiently
• Familiarity with safety protocols, infection control, and mobility equipment
