Roles & Responsibilities

• Provide compassionate direct nursing care to residents in a long-term care and rehab setting
• Administer medications and treatments accurately according to facility policies
• Perform thorough assessments and document patient status in the chart
• Collaborate with nurses, therapists, and providers to create and follow individualized care plans
• Monitor and report changes in condition promptly to ensure resident safety
• Assist with admissions, transfers, and discharge planning when needed
• Support infection control practices and maintain a safe, clean environment
• Educate residents and families with clear, empathetic communication

Qualifications

• Active Licensed Practical Nurse (LPN) licensure (New Mexico licensure preferred)
• Current Basic Life Support (BLS) certification
• Comfortable working 12+ hour day shifts and weekends as scheduled
• Prior experience in long-term care, skilled nursing, or rehab settings preferred
• Strong clinical judgment, time management, and teamwork skills
• Up-to-date immunizations and ability to meet facility onboarding requirements

Education

• Graduate of an accredited Practical Nursing program or equivalent vocational nursing diploma

Knowledge & Skills

• Medication administration and documentation best practices
• Wound care basics and skin integrity monitoring
• Resident assessment and vital signs interpretation
• Electronic medical record (EMR) charting and reporting
• Infection prevention and standard precautions
• Effective communication with interdisciplinary teams and families
