Roles & Responsibilities

• Provide skilled, compassionate nursing care to residents in a rehabilitation setting
• Administer medications and treatments accurately and document care thoroughly
• Monitor patient status and promptly report changes to the interdisciplinary team
• Support patient recovery with goal-directed care and patient/family education
• Maintain accurate, timely charting and follow facility protocols and infection-control standards
• Collaborate with therapists, physicians, and support staff to deliver coordinated care

Qualifications

• Active Licensed Practical Nurse (LPN) license in good standing
• Current BLS/CPR certification (or ability to obtain upon hire)
• Comfortable working overnight shifts and managing a rehabilitation caseload
• Strong communication skills and the ability to work as part of a multidisciplinary team
• Prior experience in rehab or long-term care preferred but motivated clinicians are encouraged to apply

Education

• Graduate of an accredited practical nursing program
• LPN diploma or certificate required

Knowledge & Skills

• Patient assessment and clinical observation skills
• Medication administration and documentation best practices
• Familiarity with wound care principles and basic restorative nursing
• Proficiency with electronic medical records and accurate charting
• Infection control, safety protocols, and regulatory compliance awareness
• Time management, prioritization, and strong teamwork
