Roles & Responsibilities

• Provide skilled physical therapy in patients' homes, delivering compassionate, individualized care
• Assess mobility, function, and home safety, then develop and implement effective treatment plans
• Document evaluations, progress, and discharge plans accurately; Epic experience preferred for documentation
• Collaborate with interdisciplinary care teams to coordinate patient-centered goals and transitions
• Educate patients and caregivers on safe mobility, therapeutic exercises, and fall prevention
• Participate in Trinity Specialty Specific Competency Testing and complete the Trinity Skills Checklist
• Maintain required licensure, certifications, and immunization/health requirements to meet facility standards

Qualifications

• Valid Physical Therapist license or ability to obtain licensure in Iowa (License/Certificate Verification required)
• BLS certification (required)
• Epic experience preferred but not required
• Successful completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
• Ability to meet health and screening requirements including fit test, 10-panel drug screen, MMR, Hepatitis B, 2-step PPD or IGRA, Varicella, Tdap, COVID-19 vaccine, and influenza vaccine

Knowledge & Skills

• Home Health care delivery and principles
• Physical Therapy assessment and treatment planning
• BLS and basic emergency response
• Familiarity with Epic for clinical documentation (preferred)
• Comfort with competency-based testing and skills checklists
• Strong communication and patient education skills
