Roles & Responsibilities

Conduct thorough psychosocial assessments to understand patients' strengths, needs, and barriers to care
Develop individualized, evidence-based treatment plans that promote recovery and resilience
Deliver therapeutic interventions across the lifespan, addressing depression, anxiety, trauma, substance use, and adjustment disorders
Collaborate closely with primary care providers, nurses, and behavioral health colleagues to coordinate whole-person care
Document clinical encounters and treatment progress clearly and timely to support continuity of care
Advocate for underserved and rural populations, connecting patients with community resources and supports

Qualifications

Master's in Social Work (MSW) from an accredited program
Active LISW license in Iowa (required)
Minimum of 2 years of clinical social work experience preferred
Experience or comfort working in rural or medically underserved settings is a plus
Strong interpersonal, organizational, and documentation skills

Education

Master's in Social Work (MSW) from an accredited program

Knowledge & Skills

Skilled in psychosocial assessment and treatment planning
Proficient in evidence-based therapeutic interventions and behavioral health approaches
Experience with trauma-informed care and substance use treatment approaches
Ability to collaborate effectively with integrated care teams
Excellent clinical documentation and case management skills
