Roles & Responsibilities

• Provide compassionate direct patient care consistent with LPN scope and facility protocols
• Administer medications and treatments accurately and document per policy
• Monitor and record vital signs, reporting changes to the care team promptly
• Collaborate with RNs and interdisciplinary team members to support individualized care plans
• Maintain accurate and timely charting to support continuity of care
• Assist with admissions, discharges, and patient transfers while ensuring safety and comfort

Qualifications

• Current LPN license (must be in good standing)
• Completion of an accredited practical nursing program
• Basic Life Support (BLS) certification preferred or willing to obtain
• Comfort working overnight shifts and collaborating within a long-term care team
• Ability to pass facility background checks and required health screenings

Education

• Graduate of an accredited LPN practical nursing program
• High school diploma or GED required

Knowledge & Skills

• Strong knowledge of medication administration and dosage calculations
• Proficient in basic wound care and restorative nursing tasks
• Skilled in patient assessment, vital signs, and documentation
• Familiarity with infection control practices and resident safety protocols
• Clear communication skills and a collaborative attitude with interdisciplinary teams
