Roles & Responsibilities

• Evaluate patients to identify functional needs and goals
• Develop individualized treatment plans that promote independence in daily living activities
• Deliver occupational therapy interventions focused on improving ADLs and functional mobility
• Collaborate with a multidisciplinary team to coordinate patient-centered care
• Document treatment plans and progress clearly and promptly in the patient record
• Educate patients and caregivers on adaptive strategies, home programs, and safety
• Adjust interventions based on clinical reasoning and patient response

Qualifications

• Active California Occupational Therapy license or eligibility to obtain one
• Graduate of an accredited Occupational Therapy program
• Strong clinical reasoning and patient evaluation skills
• Excellent communication and documentation abilities
• Passion for patient-centered care and working in a collaborative SNF setting

Education

• Graduate of an accredited Occupational Therapy program

Knowledge & Skills

• Skilled in patient evaluation and treatment planning for ADLs
• Experience delivering OT interventions in a skilled nursing facility preferred
• Proficient clinical documentation and progress note writing
• Comfortable educating patients and caregivers on adaptive techniques
• Ability to work well within a multidisciplinary care team
• Basic familiarity with common SNF workflows and electronic health records
