Roles & Responsibilities
- Provide compassionate hands-on care to rehab patients, assisting with activities of daily living
- Support the therapy team with transfers, ambulation and exercise assistance
- Monitor and accurately document vital signs, intake/output and patient status
- Communicate clearly with nurses, therapists and families to ensure coordinated care
- Maintain a safe, clean environment and follow infection control practices
- Respond promptly to patient needs and contribute to an uplifting rehab atmosphere
Qualifications
- Active CNA or GNA (or equivalent) certification in Maryland, or ability to obtain
- Current BLS (CPR) certification preferred or willingness to obtain
- Previous experience in long-term care, skilled nursing or rehab preferred (1+ year helpful)
- Strong communication skills, dependable, and a team-player attitude
- Ability to perform patient transfers and stand for shifts as required
Education
- High school diploma or GED required; CNA/GNA certification required
Knowledge & Skills
- Assisting with ADLs (bathing, dressing, grooming)
- Safe patient transfer and positioning techniques
- Basic vital signs measurement and documentation
- Familiarity with infection control and patient safety best practices
- Comfortable using electronic charting / EHR systems
- Empathy, patience, and excellent interpersonal skills
