Roles & Responsibilities

• Evaluate patients and create individualized, meaningful treatment plans
• Provide purposeful therapy to help patients regain independence in daily living activities
• Collaborate closely with nurses, therapists, and care teams to deliver coordinated care
• Document patient progress clearly and adjust interventions based on clinical response
• Educate patients and caregivers on adaptive strategies, home programs, and exercises
• Advocate for patient-centered goals and support safe transitions of care

Qualifications

• Active California Occupational Therapy license or eligibility to obtain licensure
• Graduate of an accredited Occupational Therapy program
• Strong communication skills and excellent clinical reasoning
• Passion for delivering compassionate, patient-centered rehabilitation
• SNF experience preferred but motivated new grads and seasoned clinicians are encouraged to apply

Knowledge & Skills

• Occupational Therapy assessment and treatment planning
• Clinical reasoning and evidence-based practice
• Patient evaluation for ADLs and functional independence
• Skilled documentation and progress note writing
• Patient and caregiver education on adaptive strategies and therapeutic exercises
• Interdisciplinary collaboration within a long-term care or SNF setting
