Roles & Responsibilities

• Provide compassionate direct resident care and assist with activities of daily living
• Administer medications and treatments accurately and on time
• Monitor and document vital signs, changes in condition, and care provided
• Communicate effectively with residents, families, and the interdisciplinary team
• Support safe care transitions and follow established care plans
• Respond to resident needs and emergencies in a calm, professional manner
• Contribute to a positive, resident-centered culture and continuous improvement

Qualifications

• Current LPN license in West Virginia (or compact state with WV eligibility)
• Minimum one year of recent long-term care or skilled nursing experience preferred
• Current Basic Life Support (BLS) / CPR certification
• Up-to-date immunizations and ability to pass background and health screenings
• Reliable, professional, and able to work overnight shifts

Education

• Graduate of an accredited practical nursing program (LPN/LVN diploma or certificate)

Knowledge & Skills

• Strong clinical assessment and medication administration skills
• Familiarity with documentation and electronic medical records
• Infection control practices and safe patient handling
• Clear communication and teamwork skills
• Ability to prioritize, multitask, and maintain attention to detail
