Roles & Responsibilities
- Provide safe, compassionate nursing care to residents in a rehab setting
- Administer medications and treatments accurately and timely
- Coordinate with RNs, therapists, and interdisciplinary team to support resident goals
- Monitor and document resident condition, changes, and responses to care
- Support discharge planning and assist with resident and family education
- Maintain a clean, safe environment and adhere to facility policies and standards
Qualifications
- Active LPN license in New Mexico (required)
- Current basic life support (BLS) or CPR certification (preferred or obtain upon hire)
- Previous experience in long-term care or rehab preferred
- Clear background checks and ability to meet onboarding requirements
- Strong communication skills and ability to work in a team-focused environment
Education
- LPN diploma or certificate from an accredited program; high school diploma or equivalent required
Knowledge & Skills
- Medication administration and documentation
- Wound care basics and monitoring of skin integrity
- Familiarity with electronic medical records and charting
- Ability to triage, escalate concerns, and follow care plans
- Comfort working with rehabilitative disciplines and following therapy plans
