Roles & Responsibilities
- Provide compassionate, hands-on patient care as a Licensed Practical Nurse at Bear Canyon Rehab Center
- Administer medications, monitor vital signs, and document patient responses accurately and promptly
- Collaborate with RNs, therapists, and physicians to implement and follow individualized care plans
- Educate patients and families about care routines and discharge instructions in a clear, caring way
- Recognize changes in patient condition and escalate concerns to the care team to ensure safety
- Maintain accurate, timely charting and comply with facility policies and infection control standards
Qualifications
- Active LPN license in New Mexico (required)
- Current BLS/CPR certification (required)
- Minimum 1 year of clinical LPN experience preferred; long-term care or rehab experience a plus
- Up-to-date immunizations and TB screening per facility policy
- Strong communication skills and a collaborative team mindset
Education
- Graduate of an accredited LPN/LVN program; high school diploma or equivalent
Knowledge & Skills
- Medication administration, wound care, and vital sign monitoring
- Electronic health record documentation and timely charting
- Patient education and discharge planning basics
- Basic assessment, triage, and critical thinking skills
- Infection control practices and universal precautions
