Roles & Responsibilities
- Provide compassionate, resident-centered care to geriatric patients, helping with activities of daily living and comfort measures
- Document resident care accurately and promptly in the Matrix EMR to keep the care team aligned
- Support a safe, clean environment and observe infection control and safety procedures
- Communicate effectively with the DON, charge nurse, and interdisciplinary team about resident needs and changes
- Build trusting relationships with residents and families to promote dignity and quality of life
- Participate in unit orientation and training to stay confident and competent in your role
Qualifications
- Active CNA certification required (must be eligible to submit credentials)
- BLS (Basic Life Support) certification required
- Minimum 2 years of relevant experience in long term care preferred
- Comfort working with geriatric residents and familiarity with SNF/LTC care
- Ability to pass required background and health screenings
- Must comply with radius/employment restrictions (no full-time/part-time/PRN with CommonSpirit-affiliated facilities)
Education
- High school diploma or equivalent; CNA certification required
Knowledge & Skills
- Hands-on experience with resident care, ADLs, and mobility support
- Familiarity with Matrix EMR or similar charting systems
- Strong communication and teamwork skills
- Basic knowledge of infection control, safety protocols, and elder care
- Ability to follow care plans and escalate clinical concerns appropriately
- Dependability and professionalism for a 12-hour shift environment
