Roles & Responsibilities

• Assist patients with activities of daily living such as bathing, dressing, grooming, and toileting with dignity and compassion
• Support mobility and transfers safely, using proper body mechanics and assistive devices
• Monitor and report changes in patient condition, including vital signs and behavior, to the nursing team
• Collaborate with rehab therapists to reinforce therapy goals and encourage patient participation
• Provide warm, person-centered care that promotes comfort and emotional well-being
• Maintain accurate documentation of care, observations, and tasks completed
• Follow infection control protocols and facility policies to keep patients safe

Qualifications

• Active GNA or equivalent state nursing assistant certification
• Current BLS/CPR certification or willingness to obtain upon hire
• Previous experience in long term care or rehab preferred but new grads with strong clinical skills are welcome
• Ability to pass background check, drug screen, and meet facility immunization requirements
• Reliable, punctual, and comfortable working as part of a multidisciplinary team

Education

• High school diploma or GED required
• GNA certification or state equivalent required

Knowledge & Skills

• Strong patient care skills and comfort assisting with ADLs
• Knowledge of basic vital sign measurement and reporting
• Familiarity with infection control practices and safety protocols
• Good communication skills with patients, families, and clinical staff
• Basic documentation skills; experience with electronic charting a plus
• Ability to prioritize tasks and manage time during a busy shift
