Roles & Responsibilities

• Provide skilled physical therapy evaluations and hands-on treatment to help residents maximize mobility and independence
• Develop individualized care plans that set clear, measurable goals
• Document daily progress and outcomes accurately and timely
• Collaborate with the interdisciplinary team to coordinate care and improve resident outcomes
• Educate patients and caregivers on mobility, safety, and home strategies to support recovery

Qualifications

• Active California Physical Therapist (PT) license required
• Experience in skilled nursing facility (SNF) settings preferred
• Strong documentation skills and attention to detail
• Excellent communication and clinical reasoning abilities
• Ability to collaborate effectively with rehab and nursing teams

Education

• Doctor of Physical Therapy (DPT) or Master of Physical Therapy preferred

Knowledge & Skills

• Physical Therapy and PT treatment techniques
• Care planning and outcome-focused care
• Daily clinical documentation and progress note writing
• Interdisciplinary collaboration and patient education
• Clinical reasoning and resident safety/mobility best practices
• Clear professional communication with residents, families, and staff
