Roles & Responsibilities
- Communicate promptly with physicians, families, and care teams about significant changes in resident condition
- Complete resident assessments accurately and on schedule to ensure individualized care plans
- Administer medications following the 5 Rights and maintain impeccable medication records
- Perform wound care and clinical assessments as needed to promote healing
- Support CNAs by answering call lights and helping prioritize resident needs
- Monitor residents for changes in condition and intervene or escalate when necessary
- Lead the floor by assessing situations, directing care, and mentoring team members
- Report any allegations of abuse, neglect, or misappropriation immediately to DON and Administrator
- Maintain thorough, accurate charting and resident records to ensure continuity of care
Qualifications
- Current Wisconsin Registered Nurse (RN) license required — must be maintained
- Associate degree in Nursing required; BSN preferred
- Current CPR/BLS certification required
- 1+ years of related nursing experience preferred, especially in long-term care or skilled nursing
- Strong attention to detail and exceptional accuracy in clinical tasks and documentation
- Ability to lead on the floor, communicate clearly, and collaborate with interdisciplinary teams
- Willingness to follow facility policies (including scrub color requirement: Navy Blue)
Education
- Associate Degree in Nursing (required)
- Bachelor of Science in Nursing (preferred)
Knowledge & Skills
- Medication administration and adherence to the 5 Rights
- Resident assessment and care planning
- Wound care techniques and documentation
- Strong charting and accurate record-keeping
- Effective communication with residents, families, and clinical teams
- Leadership and ability to direct CNAs and coordinate care
- CPR/BLS life support knowledge
