Roles & Responsibilities

• Coordinate patient care across the continuum, ensuring seamless transitions from hospital to home or other care settings
• Assess patients’ medical, psychosocial, and discharge needs to create individualized care plans
• Develop and implement case management interventions that improve outcomes and reduce readmissions
• Collaborate with physicians, social workers, therapists, and community resources to secure necessary services
• Advocate for patients and families to ensure timely access to care and appropriate follow-up
• Document comprehensive assessments, care plans, and progress notes in the medical record
• Educate patients and caregivers about diagnoses, treatments, and post-discharge care plans

Qualifications

• Valid RN license in Texas or eligibility for Texas licensure
• Minimum 1 year of relevant nursing experience (case management experience preferred)
• Current BLS (Basic Life Support) certification or equivalent
• Excellent clinical assessment and critical thinking skills
• Strong communication and interdisciplinary collaboration abilities
