Roles & Responsibilities

• Provide compassionate, patient-centered nursing care in a rehabilitation setting
• Administer medications and treatments safely and accurately
• Monitor and document vital signs, patient status, and changes in condition
• Collaborate with therapists, physicians, and support staff to coordinate care plans
• Assist with activities of daily living and support patients' progress toward goals
• Educate patients and families about care plans, medications, and discharge instructions
• Respond promptly to emergencies and escalations while maintaining calm and clear communication
• Maintain accurate, timely charting in the electronic health record

Qualifications

• Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license (required)
• Current BLS/CPR certification (required) — ACLS or specialty certifications a plus
• Experience in rehab or long-term care is preferred but supportive training is available
• Strong clinical assessment, medication administration, and documentation skills
• Ability to pass background check, drug screen, and meet facility onboarding requirements
• Excellent teamwork and communication skills; compassionate bedside manner

Education

• Graduate of an accredited practical nursing program (for LPN) or accredited nursing program (ADN/BSN for RN preferred)

Knowledge & Skills

• Skilled in medication administration and nursing assessments
• Comfortable working overnight shifts and managing patient acuity during nights
• Familiarity with rehab care plans and restorative nursing practices
• Proficient with electronic health record documentation
• Strong time management and prioritization in a fast-paced environment
• Clear communicator with patients, families, and the interdisciplinary team
