Roles & Responsibilities

• Provide skilled physical therapy evaluations and develop patient-centered treatment plans
• Deliver hands-on and therapeutic interventions to improve patient mobility and function
• Monitor patient progress, update goals, and modify treatment plans as needed
• Educate patients and families on exercises, home programs, and fall prevention strategies
• Thoroughly document evaluations, treatments and discharge summaries in the EMR
• Collaborate with interdisciplinary team members to ensure seamless care and safe transitions
• Participate in team meetings and contribute to a positive, patient-focused clinical culture

Qualifications

• Current and unrestricted Physical Therapist license in New Hampshire, or ability to obtain prior to start
• Graduate of an accredited physical therapy program (DPT preferred)
• Basic Life Support (BLS) certification required; additional certifications a plus
• Comfortable with documentation in electronic medical records and standard PT billing practices
• Strong communication skills and the ability to work collaboratively across disciplines
• Previous outpatient/inpatient/rehab experience preferred

Education

• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise and manual techniques
• Knowledge of mobility, gait training, orthotic/prosthetic considerations and ADL adaptation
• Competent with therapeutic modalities and assistive devices
• Excellent clinical reasoning and goal-setting skills
• Effective documentation, time management and caseload prioritization
• Ability to educate patients and caregivers clearly and compassionately
