Roles & Responsibilities

• Provide high-quality, compassionate nursing care to residents at the rehab center
• Administer medications and treatments safely and accurately
• Assess patients and communicate changes promptly to the interdisciplinary team
• Document care thoroughly and maintain accurate medical records
• Assist with care planning and support resident rehabilitation goals
• Collaborate with therapists, CNAs, and physicians to deliver coordinated care
• Support a positive, resident-centered environment that promotes dignity and independence

Qualifications

• Active LPN or RN licensure in good standing
• Current BLS/CPR certification
• Previous experience in long-term care, skilled nursing, or rehab settings preferred
• Strong clinical assessment and medication administration skills
• Excellent communication and teamwork abilities
• Up-to-date immunizations and ability to pass facility background checks

Education

• Graduate of an accredited Practical Nursing program for LPNs or an accredited nursing program for RNs

Knowledge & Skills

• Knowledge of nursing assessment, care planning, and medication administration
• Familiarity with wound care, IV/PO medication routes, and basic rehab nursing practices
• Comfortable using electronic medical records and documentation systems
• Ability to prioritize tasks in a fast-paced clinical environment
• Strong interpersonal skills for working with residents, families, and the care team
