Location: Uniontown, Pennsylvania

Contract Type: Per-Diem

Shift Type: Per Diem (as-needed), 3:00 PM - 11:00 PM, Day shift (evening/PM coverage as scheduled)

Hours per Week: 40

Candidate Pay Rate: $21 per hour

Roles & Responsibilities

Provide compassionate personal care and assistance with activities of daily living
Assist patients with bathing, dressing, toileting, and mobility safely
Take and record vital signs and report changes in patient condition
Document care accurately and communicate effectively with the nursing team
Support a clean, safe, and comfortable environment for residents
Build trusting relationships with patients and their families through respectful interactions

Qualifications

Current CNA certification (Class: CNA) — required
Valid state certification or eligibility to obtain Pennsylvania CNA certification
Ability to pass background check and required health screenings
Comfortable working in a fast-paced long-term care environment
Strong communication skills and a team-oriented attitude

Education

High school diploma or GED preferred

Knowledge & Skills

Skilled in assisting with ADLs and mobility support
Proficient in taking and recording vital signs accurately
Basic charting and documentation skills
Knowledge of infection control and safe patient handling
Compassionate patient interaction and strong interpersonal skills
