Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to assigned residents
• Administer medications and treatments safely and accurately
• Assess and document patient conditions, vital signs, and changes in status
• Collaborate with nurses and interdisciplinary team members to coordinate care
• Support patient and family education with clear, caring communication
• Maintain accurate clinical documentation in the facility charting system
• Follow infection control protocols and facility policies to ensure safety

Qualifications

• Current, active LPN license in good standing (LPN required; RN candidates also accepted as noted)
• Current BLS/CPR certification or willingness to obtain upon hire
• Previous clinical experience in long-term care, rehab, or similar settings preferred
• Strong communication skills and ability to work effectively as part of a team
• Ability to pass standard background checks and drug screening

Education

• Graduate of an accredited Practical Nursing program or equivalent

Knowledge & Skills

• Skilled in medication administration and basic nursing procedures
• Comfortable with assessments, wound care basics, and vital sign monitoring
• Proficient in accurate charting and clinical documentation
• Ability to prioritize tasks and manage time during busy shifts
• Empathetic patient interaction and family communication skills
