Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents on 12-hour day shifts
• Administer medications and treatments accurately and document thoroughly
• Assess resident condition, monitor vital signs, and report changes to the care team
• Collaborate with RNs, therapists, and interdisciplinary staff to implement care plans
• Perform wound care, support activities of daily living, and promote resident dignity
• Educate residents and families about care, safety, and discharge planning as needed
• Maintain accurate electronic and paper records, following facility and regulatory standards
• Support a positive, team-focused environment and contribute to continuous improvement

Qualifications

• Current LPN license (Licensed Practical Nurse) — state licensure required or ability to obtain WV licensure promptly
• T-LPN credential or travel LPN experience preferred
• Current BLS/CPR certification (or willing to obtain prior to start)
• Comfortable with 12-hour day shifts and weekend-focused scheduling
• Strong interpersonal skills and ability to work collaboratively with a multidisciplinary team
• Up-to-date immunizations and ability to pass facility onboarding requirements

Education

• Completion of an accredited Practical Nursing program (LPN diploma or certificate)

Knowledge & Skills

• Medication administration and safe documentation practices
• Basic wound care, vitals monitoring, and resident assessment
• Familiarity with long-term care or skilled nursing workflows
• Solid time management, prioritization, and delegation skills
• Comfort using electronic medical records and standard clinical charting
• Empathy, patient advocacy, and strong communication with residents and families
