Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents and patients
• Administer medications and treatments accurately and document care promptly
• Perform routine assessments and communicate changes in condition to the care team
• Collaborate with nurses, therapists, and support staff to coordinate patient-centered care
• Assist with admissions, discharges, and transitions while maintaining resident dignity
• Maintain clear, timely documentation in the patient record and follow facility policies

Qualifications

• Active LPN license (practical/vocational nursing) appropriate for Pennsylvania or eligible to obtain
• Current CPR/BLS certification or willingness to obtain upon hire
• Comfortable with medication administration, wound care basics, and routine nursing procedures
• Able to work evenings and adapt to the pace of a busy long-term care facility
• Professional, patient-focused attitude with good communication and teamwork skills
• Prior experience in long-term care, rehab, or similar settings preferred

Education

• Graduate of an accredited LPN / Practical Nursing program
• High school diploma or GED required

Knowledge & Skills

• Strong patient assessment and observation skills
• Medication administration and basic clinical procedures
• Accurate charting and documentation practices
• Good communication with patients, families, and interdisciplinary teams
• Infection control and safety-conscious mindset
• Ability to prioritize tasks and manage time during a full shift
