Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients
• Administer medications safely and accurately, following facility protocols
• Monitor and record vital signs and report changes promptly to the care team
• Assist with activities of daily living (ADLs) and support patient comfort and dignity
• Document care thoroughly in the patient record and maintain clear communication with coworkers
• Collaborate with nurses, therapists, and other staff to develop and follow care plans
• Contribute to a safe, clean environment by following infection control and safety procedures

Qualifications

• Active Licensed Practical Nurse (LPN) license (state licensure required or eligible to obtain)
• Current basic life support (BLS/CPR) certification (or willing to obtain)
• Experience in long-term care, skilled nursing, or similar patient-focused settings preferred
• Strong communication skills and a collaborative, team-first approach
• Dependable, professional, and committed to high-quality patient care

Education

• Vocational nursing diploma or equivalent practical nursing program completion

Knowledge & Skills

• Skilled in patient assessment and monitoring of vital signs
• Medication administration and documentation best practices
• Comfortable assisting with ADLs and mobility support
• Experience with electronic health records (EHR) documentation
• Good infection control practices and safety awareness
• Ability to communicate clearly with patients, families, and interdisciplinary teams
